Florida Statute 1003.22 and Rule 64D-3.046 of the Florida Administrative Code, and school board policy 5320 mandates that children entering, attending, or transferring into school receive the appropriate grade level vaccines.
The Florida Department of Health in Miami-Dade County would like to inform parents/guardians who have students in PRE-KINDERGARTEN, SIXTH grade, and students with an EXPIRED Immunization status that their child/children are due for grade appropriate vaccines.
- Students entering Kindergarten Fall 2022 must obtain the 2nd MMR (Mumps Measles and Rubella) and the 2nd Varicella vaccine.
- Students entering the 7th grade Fall 2022 must obtain the Tdap booster (tetanus diphtheria pertussis) vaccine.
- Students in ALL grades with an expired immunization status must present a new Florida Certificate of Immunization-680 form.
- Students will not be permitted into school without the appropriate vaccine requirements.
